Identifying the root causes of an incident or event can be a relatively complicated process, the investigation often reveals several causal factors. The concept behind incident investigation is to learn the lessons which lead to the incident so as to prevent a recurrence.

Managers should regard incident findings which are limited to, "No Take 5" or "No JSA" as flawed and a failure on the part of the investigator to apply due diligence by investigation thoroughly the chain of events leading to the incident.
